What BCBSTX asks for in an ABA request
BCBSTX’s ABA clinical service request form asks for:
- An autism spectrum disorder diagnosis made within the past 36 months by a primary care physician (family practice, internal medicine, or pediatrics), a developmental-behavioral or neurodevelopmental pediatrician, a child neurologist, a psychiatrist, or a licensed clinical psychologist.
- A skills assessment completed within 30 days of the request, using one standardized tool (such as the VB-MAPP, ABLLS, AFLS, ABAS, or Vineland) for the whole episode of care.
- A treatment plan with measurable goals, weekly parent training with measurable parent goals, and a plan for fading services and discharge.
- Sessions delivered by trained, background-checked staff under regular supervision by a qualified behavior analyst.
BCBSTX authorizes a re-assessment every six months, which is when your BCBA updates goals and submits progress data to continue services.
Prior authorization with BCBSTX
BCBSTX states that ABA services require prior authorization, and claims filed without one are denied. For some members, behavioral health benefits are administered by a vendor instead of BCBSTX itself, which the provider confirms before submitting. Our team assists with these requests once intake is complete.
Age, dollar, and visit limits
BCBSTX’s ABA program document states that, effective January 1, 2021, it removed exclusions and limitations on ABA for autism, with no age, dollar, or visit limits. The same document notes that self-funded employer groups administered by BCBSTX may opt out of ABA coverage, so your employer’s plan may differ.
Employer plans, teachers, and state employees
The Texas Department of Insurance regulates fully insured plans; self-funded employer plans follow federal law (ERISA) and set their own benefits. If your member ID card shows “TDI” or “DOI,” the plan is fully insured. TRS-ActiveCare (Texas public school employees) and ERS (Texas state employees) plans have their own BCBSTX ABA request forms, and Texas’s autism coverage law extends to TRS-ActiveCare.
For state-regulated group plans, Texas Insurance Code §1355.015 requires coverage of generally recognized autism treatment, including ABA, for a child whose diagnosis was in place before their 10th birthday. For enrollees age 10 and older, those plans are not required to cover more than $36,000 of ABA a year. Many plans cover more than the law requires, which is why we check your specific plan.
Federal employees (FEP Blue)
The Blue Cross and Blue Shield Service Benefit Plan for federal employees requires prior approval for ABA and all related services, including assessments, and excludes ABA provided as part of an educational program or by a school. In 2026, FEP Blue Focus limits ABA to 200 hours per person per calendar year.
BCBSTX Medicaid (STAR and STAR Kids)
BCBSTX STAR and STAR Kids are Texas Medicaid plans with their own network of ABA providers. Texas Medicaid covers ABA for members age 20 and under. If your child has one of these plans, call the member services number on the ID card to find an in-network ABA provider, or call us to ask whether we can help.
How Vivid handles your BCBSTX benefits
We verify eligibility, network status, and cost share, review known costs with you, collect the diagnosis report your plan requires, complete the BCBA assessment once it is authorized, and assist with the treatment authorization request. The full process is on our How to Get Started page. Insurance verification and authorization do not guarantee payment; coverage is determined by your plan’s terms.
